Disability Courtesy Shuttle Acknlowledgement
All participants must review and acknowledge the Courtesy Shuttle Guidelines in order to be eligible for this service.
Main Campus Operating Hours are 7:30am – 4:00pm Monday thru Friday
CSU Courtesy Shuttle operates on a scheduled, shared-ride basis for actively enrolled students who have a documented permanent mobility disability that limits their ability to walk prescribed distances.
Documentation The CSU Courtesy Shuttle participants are required to provide medical need documentation from their physician. In order to comply with Federal HIPAA privacy law requirements, you will be required to sign a medical records release which includes the required medical need documentation for your doctor to complete and return. Program staff, through this acknowledgement, may contact your physician.
Emergency Room discharge documents or prescriptions
are not
accepted forms of medical need documentation.
Participants must provide their
physician’s full name, office fax number, and the office address
.
The documentation is sent via a secure fax server and must be completed and received at our office 5 business days prior to requested start date. If the documentation has not been returned within the time frame allowed, you will be notified by email. It will be your responsibility to contact your doctor and follow up on your documentation.
CSU Courtesy Shuttle Service Area Limitations All Courtesy Shuttle rides will begin and end at or near a CSU building, CSU transit station or CSU parking lot/structure owned by CSU on the Main Campus. The Foothills and South campuses receive ADA Compliant transit provided by the City of Fort Collins Transfort. The Courtesy Shuttle does
not
make stops at privately held apartments, condo complexes, or businesses.
Scheduling and Pick-up/Drop-off
Rides from residence halls, parking lots/structures are scheduled with a “
lead time
” as early as 45 minutes before your scheduled class begins**.
**Your
lead time
or any other ride time may be adjusted in order for us to maximize vehicle occupancy. We will notify you when this is necessary.
Each building and parking lot/structure on campus has a designated drop-off and pick-up location generally near a sidewalk or accessible curb. Due to accessibility logistics, there are a few buildings with more than one location. Some buildings require a short distance of travel to get to the nearest pickup/drop off location. The location will be assigned by the Courtesy Shuttle driver. Drivers will always remain with the vehicle and will not enter buildings to assist passengers or assist passengers to and from the pickup and drop off locations.
Pick-up times for those going from one class immediately to another class will receive first priority and will be scheduled as close to the time your previous class ends as possible. Pickup time for those not going directly to another class will be scheduled as close to the end of your previous class as possible, while prioritizing those going immediately to class. You will be provided ample time to exit the building but are encouraged to do so as quickly as possible. 5-10 minutes is the average time needed to exit most campus buildings.
Your driver may be assigned several scheduled pick-ups at the same time at multiple locations and are generally close together. Depending on where you are on your driver’s route, you could be the first or last pick-up on the route.
Our drivers are assigned rides according to the most efficient route. Any changes to the pick-up or drop-off location must be requested through your driver 3 business days in advance of the schedule change. The program cannot guarantee a seat should you change your scheduled trips times and locations.
There is a 90-minute break minimum for anyone who has a break between classes and would like to return to your residence hall or parking location. The 90-minute break is determined by the scheduled time of your previous class to your residence hall/parking to what would be the scheduled “lead time” from your residence hall/parking location
Mandatory Restrictions
The Courtesy Shuttle is equipped to transport mobility devices and other larger assistive equipment if space is available on the vehicle for those deemed eligible for the service. A mobility device will restrict the number of passengers during the assigned trip due to spaces constraints necessary to carry the mobility device. The Courtesy Shuttle programs reserves the ability to restrict the number of passengers when a mobility device trip is scheduled. Drivers can provide only minimal physical assistance boarding and off boarding.
For safety purposes, riders with a
service animal
will always occupy the front seat. Service animals must never pose a safety hazard to anyone in the cart or to itself.
The CSU Courtesy Shuttle reserves the right to terminate access to a service animal if the handler fails to maintain control of the animal.
No-Show and Cancellations Courtesy Shuttle participants are required to call when not attending class/work for the day or not needing a particular ride to class due to a class cancellation by calling the driver’s number provided to you. The definition of a no-show is a passenger who schedules a ride and does not take the ride or does not cancel it.
Please note, the return trip of all missed rides are also canceled.
If you:
Demonstrate a pattern for being late for rides
– The Courtesy Shuttle may suspend service for a semester in cases where an approved individual establishes a pattern or practice of non-canceled scheduled trips. Said individuals will receive specific written notice of a possible upcoming suspension, citing the reason for the suspension, and will be given an opportunity to appeal the suspension before it takes effect. If you wish to resume service following a suspension, your request will be subject to availability and the first-come, first-served structure of the service.
Fail to call or miss rides for
2 consecutive days, the passenger will receive written notice of suspension and be given an opportunity to appeal, before their service is suspended for the semester.
Fail to call and no-show your first ride of the day -
All rides suspended for the day
