Disability Courtesy Shuttle CSU provides a supplemental, first-come, first-served Courtesy Shuttle for students with an eligible permanent ambulatory disability. Ambulatory disabilities are those which significantly impact the ability to walk.
Step 1: Shuttle Application Complete the Temporary Disability Courtesy Shuttle Service Application form. Information on the form must be accurate. All boxes must be completed, blank fields will void application. *Upon completion of the shuttle application form, please contact the CSU Student Disability Center and register if you haven’t already. The process cannot move forward beyond this point without being registered with the SDC
Step 2: Verification of Mobility Impairment. Fill out the student portion of the Mobility Impairment Verification Form and ensure all fields are completed accurately. Once the form is submitted, the document will be sent to the Colorado State University Student Disability Center for review. Upon completion, it will be returned to Fleet Services for final confirmation.
